Shoulder replacement surgery (also known as shoulder arthroplasty) is a procedure in which the surgeon removes damaged cartilage and bone from the shoulder joint and replaces it with a replica implant. Once healed, the implant will help the shoulder rotate and glide with ease, both reducing pain and restoring function. All shoulder surgeries currently take place at either York Hospital or Portsmouth Regional Hospital, where our surgical teams have staff privileges. Beginning in the Spring of 2021, we will also offer select shoulder surgery procedures at our new ambulatory surgical center located on-site at our offices in Portsmouth, NH.

Shoulder Replacement Candidates

Shoulder replacement surgery is most common in adults 50 years or older and most patients have been suffering from severe pain, degenerative joint disease, arthritis, or other shoulder conditions that are affecting their shoulder function and quality of life. All shoulder surgery candidates are those who have not, do not, or will not be effectively treated through non-operative measures like rest, physical therapy, or injections.

Our surgical screening process starts with a clinical evaluation of your shoulder pain and personal and family health history. Most often, advanced imaging is ordered — like x-ray, MRI, EMG, ultrasound, or CT Scans — to be sure our diagnosis is accurate. This information is combined to determine our recommended treatment course. In all cases, we will work with you, other healthcare professionals like your PCP or health services like a lab, as well as your family, to be sure everyone clearly understands the benefits, risks, and expected recovery timelines related to any treatment.

Prior to your surgery, it is crucial you meet with your orthopedic shoulder surgeon to receive a full evaluation. This evaluation may include diagnostic metrics such as X-ray, blood tests, or MRI to help best inform your care and avoid potential complications.

During shoulder surgery, you will be given the anesthesia that is most appropriate for you to prepare for the procedure. In the operating room, your surgeon will create an incision on the shoulder in order to access the damaged bone and joint. The size and location of the incision will depend on the approach used. Minimally invasive techniques, including a direct anterior or posterior approach, have been developed to minimize soft tissue damage and reduce overall recovery time. Once the bone is accessed, the surgeon will remove the damaged or diseased cartilage and bone and replace it with a replica implant.

After your shoulder replacement surgery, you will be monitored by hospital support staff until the anesthesia wears off. Some patients are eligible to go home day-of surgery.
