Dr. Akhil Sastry is a board-certified and fellowship trained orthopedic surgeon specializing in hip and knee joint replacement and robotic assisted surgery.

Widely renowned for his work with MAKO technology, Dr. Sastry is credited as the 1st Master Surgeon of Robotic Surgery in New England. He is recognized as the first surgeon to have performed a robotic-assisted total knee replacement in New Hampshire and Maine, and was among the first 15 surgeons to perform this operation worldwide. He has since performed over 1,000 robotic-assisted knee replacements.

Dr. Sastry is also a pioneer of the direct anterior approach for total hip replacements. He was the first surgeon in the Seacoast to introduce this minimally invasive, muscle-sparing approach back in 2010 and has performed over 1,500 hip replacements!

Dr. Sastry continues to educate and advance the field of robotic surgery through podium presentations, journal publications, and hands-on training.

  • Introduction

    The anterior approach allows access to the joint without detaching any muscles around the hip. The muscles are carefully retracted along the internervous plane with the assistance of a specialized “hip table”. This allows the surgeon to minimize any soft tissue trauma to the hip allowing for a faster recovery.

  • How is this different from other minimally invasive approaches?

    Traditional hip surgery splits the gluteus maximus and short external rotator muscles of the hip joint. This requires patients to follow strict “hip precautions” after surgery to prevent dislocation. Hip precautions require patients to not flex the hip beyond 90 degrees. This can make activities such as getting into a car, sitting on a toilet seat, and climbing stairs more challenging. Since the direct anterior approach leaves the muscles undisturbed, hip precautions are not required.

  • Who is a candidate for surgery?

    Patients who have been suffering from arthritis or avascular necrosis may benefit from hip surgery.

    Please note that Dr. Sastry is willing to perform the traditional hip replacement procedure if a patient is not an ideal candidate for the anterior approach.
